Each enrolled student is provided an individual developmental assessment at no additional cost.

Please click here or on the link below to download the ODLC Application Form.  To apply for your child's enrollment in our preschool, simply print the form, complete and return it by mail or in person to:

Once we receive your applications we will call you to schedule a tour, speak with our Program Director, and meet with your child.
